Medical Clearance Compliance

Once verified that the student is attending classes on campus, the following documentation is required by FDU/State of New Jersey:

All Students:

  • Authorization Form
  • Student Profile Form
  • Health History Form
  • Meningococcal Vaccination Requirement Questionnaire (Meningitis Response Form)
  • MMR (Measles/Mumps/Rubella) Vaccine Series, total of 2 doses required
  • Hepatitis B Vaccine Series, total of 3 doses required
  • Meningococcal Quad ACYW-135 Vaccine for all students ages 18 and under, booster dose must be given after 16th birthday.
  • Current Mantoux/PPD skin test or QuantiFERON-Gold blood test for tuberculosis, done within one year before starting classes at FDU.

Resident Students need to submit the following additional medical requirements:

  • Current Physical Examination Form, done within one year of starting classes at FDU
  • Meningococcal Quad ACYW-135 Vaccine required for ALL dorm students, regardless of age
  • Varicella Vaccine Series, total of 2 doses (or history of having Varicella/Chicken Pox disease)
  • Adult Tdap Vaccine, given within past 10 years

Blood work is also accepted in lieu of vaccination records for MMR, Hepatitis B and Varicella. Titers can be drawn to check for immunity – if titers show positive/reactive immunity, there is no need for additional vaccination. However, if non-immune to any of these diseases, booster vaccines are required. Entire series or one booster dose will depend on previous vaccination history.

Actual copies of laboratory reports for all blood work done, including QuantiFERON-Gold tuberculosis tests, must be submitted through the Medicat portal in order to meet compliancy requirements.

MEDICAT PORTAL ACCESS INSTRUCTIONS

To locate the Medicat portal:

1. Go to FDU Homepage: www.fdu.edu

2. Click: For Students (located in the white space next to the FDU logo)

3. Find your designated campus (Florham Campus or Metro Campus)

4. Under your designated campus, click: Health Services

5. Click: Medicat Patient Portal

6. Type Fairleigh Dickinson University under Choose Your Home Organization

7. Click: Sign Into Medicat

MEDICAT PORTAL DOCUMENTATION INSTRUCTIONS

  1. Complete all interactive forms: Health History, Student Profile, FDU Authorization, Meningococcal Vaccine Requirement Form
    • RESIDENT STUDENTS ONLY: download and print physical exam form and resident immunization record located in the Forms section
    • COMMUTER STUDENTS ONLY: download and print commuter immunization record only located in the Forms section—physical exam form is not required
  3. All students must take the printed forms to their medical provider for completion. Once completed, submit all documents into the Medicat portal.
  4. Sign back into Medicat and to the left of the screen, you will see “upload”, click it. This is where you upload your authorized medical records into the Medicat portal.
