Student Health Requirements
To ensure the health and wellness of the community and prevent communicable disease outbreaks all incoming students in the following groups
- Residential students
- Commuter students
- Nursing students
have health and vaccination requirements and must submit health records and proof of the following vaccines.

Form Submission Requirements
- All records must be received by the Office of Health Wellness by the deadlines specified below:
- EOF Students: June 10
- Student Athletes: June 24
- International Students: July 19
- All New Incoming Fall Students: August 2
- All New Incoming Spring Students: December 5
- Records must be submitted into the University Medicat Portal. For instructions about submission and medical clearance, please review this document Student Instructions-Medical Clearance Compliance.
- Students will not be permitted to register for a class or reside in FDU housing until the health forms and immunization records are completed and received.
- All forms submitted MUST be in English.
- Students who request a medical or religious exemption will be required to have blood titers drawn for Varicella, MMR, and Hepatitis B; and can choose to either have a CXR and/or a QuantiFERON TB gold test performed in lieu of a TB skin test. Please see the relevant State of NJ information.
- You will also be required to complete the exemption form provided within Medicat and if a medical exemption you are required to have documentation from your health care provider supporting the reason for the exemption.
