HRSA ANEW Application

I am aware that as part of this award, I will need to participate in 1-2 days/semester of education/training targeting the care of rural and remote patient populations.

I am aware that ANEW scholarship funds may impact my receipt of other financial aid awards, tuition waivers, or federal loans. If I have any additional financial questions or concerns, I have been directed to contact the FDU Financial Aid Office.

I am aware that alteration in my planned program pathway (e.g.: deceleration, leave of absence) may result in loss of ANEW scholarship funds.

I am aware that failure to secure clinical placement in a timely manner may result in alteration of program pathway and result in loss of the ANEW scholarship funds.

I am aware that submission of this application does not constitute award of ANEW scholarship funds. Awards will be allocated, based upon limited availability, following review of application, financial status, and academic performance/potential by the Selection Committee.

I am aware that if funded from any HHS source, I agree to maintain contact with the FDU Financial Aid Office (or school representative) for a period of no less than 5 years. The information requested will be given to HRSA, which will be used to provide justification for FDU to be awarded additional scholarship/grant funding for future awards.
